Conservative treatment of the equine-varus congenital idiopathic foot. Efficiency Evaluation

ABSTRACT
This is a clinical evaluation of the response to nonsurgical treatment of Rigid Congenital Club foot in 22 new borned (37 feet), treated by the Technique proposed by Ponseti, consisting in serial manipulations with minimal force, followed by casting. The correction of the four components of the congenital club foot (adduction, supine, varus and equinus) was evaluated by clinical criteria and with the data correction we made a graphic against time through the statistical methods: Kaplan-Meier efficiency and Survival Function tests. The supine and adduction were corrected in an average time of six weeks with 89.2% success rate. Despite varus and equinus, require about 12 manipulations and casting with low success possibilities rates of 35 and 43% respectively. In some cases, it was necessary subcutaneous enlargement of the Achilles tendon, but the response was good in just 41% of the cases, that is why we don’t recommend this procedure. Although, the success rates of nonsurgical treatment are 50%, today, is the best selection to starts the correction of the Rigid Congenital Club Foot, in order to eliminate the problem or as a preparative procedure for surgery.
