Central venous O2 saturation difference as pronostic marker of diastolic dysfunction and weaning failure

ABSTRACT
Mechanical ventilation is associated with complications and should be discontinued as soon as possible. Some patients fail to weaning despite having extubation criteria, other predictors must be studied, such as central venous oxygen saturation (SvcO
2).
Objective: To determine the delta SvcO
2 (D-ScvO
2) as a predictor of diastolic dysfunction and failure of ventilator weaning.
Material and methods: Clinical research, design: Test predicts. Conducted from March to August 2013 in ICU patients with mechanical ventilation for more than 48 hours and criteria for spontaneous breath test. Central gas analysis was performed at baseline and two hours later for measuring venous D-SvcO
2. In patients with unsuccessful weaning B-type natriuretic peptide (BNP) was measured.
Results: In the group of patients with successful weaning, the average was 2.73 ± 1.28% and in patients with unsuccessful weaning was 6.33 ± 1.37% (p = 0.001.) With a sensitivity to the D-SvcO
2 96.3% and specificity of 100%.
Conclusions: We consider the D-SvcO
2 during weaning of mechanical ventilation as a new and independent prognostic marker for the success or failure of extubation.
