ABSTRACT
Isotretinoin is a synthetic retinoid that interacts with the specific receptors of the retinoic acid. It reduces the size of the sebaceous gland, with a significance decrease in the sebaceous secretion and the cellular adhesion eliminating the follicular plugs and as a consequence also the formation of comedons. It could be the ideal drug because it has the following actions: Antiseborrheic, keratolytic, and also with an indirect effect on P. acnes and in the inflammation.It is indicated mainly in acne conglobata, fulminans and nodulocystic or in papulo-pustular resistant to the traditional treatment; other indications are rosacea, demodecidosis, gram-negative folicullitis and pyoderma facial. The recommended dose is 0.5 to 1 mg/kg of body weight, till a total dose of 120 to 150 mg/kg, which is usually achieved in 4 to 6 months. The common adverse effects are xeroderma, xerosthomia, xerophthalmia and cheilits. The teratogenic effect is the most important one, but it also has interactions with drugs such as vitamin A, tetracyclines, carbamazepine, and alcohol.
