ABSTRACT
In most patients (90-95%), hepatocellular carcinoma (HCC) is associated with liver cirrhosis, and its appearance is very rare
without a concomitant chronic liver disease. Hepatic cirrhosis is a known risk factor for the development of hepatocellular
carcinoma (HCC), as the chronic inflammation associated with regenerative processes necro condition the occurrence of genetic
mutations that ultimately lead to tumor cell development. Despite the foregoing, there is a percentage of non-cirrhotic
patients who develop the disease. The case of a male patient asymptomatic 68-year-old with a history of DM2 12 years treated
with glibenclamide diagnosis occurs, diagnosed hypertension treated with captopril 18 years ago. In checking routine detects
a liver tumor with Alfa - fetoprotein greater than 500 ng/dL with negative viral panel, it was decided to perform right hepatectomy.
The report final pathology reports a moderately differentiated hepatocellular carcinoma of 5 x 4 cm (pT2) with macrovesicular
steatosis and fibrosis in residual liver. Given the rarity of the disease, we decide to present the case.
