ABSTRACT
The xanthogranulomatous pyelonephritisis a severe and strange infection of the renal
parénquima. Their clinical manifestations in many occasions simulate a neoplasia and
the one diagnoses preoperatorio it is difficult. A case of a 68 year-old patient is
presented with an I diagnose initial of Fever of unknown origin, in which was possible
to carry out the diagnostic preoperatorio, and then to be confirmed by the study
anatomopathologic. Later to the surgery the patient had a satisfactory evolution returning
the clinical square.
