Diagnosis and Treatment of Primary Cutaneous T-cell Lymphoma

ABSTRACT
Primary cutaneous lymphoma is an heterogeneous group of non-Hodgkin lymphomas showing a considerable variation in clinical presentation, histology, immunophenotype, and prognosis.
This variety, together with a different biological behaviour from their systemic counterparts led to the development of specific classifications, like the EORTC system.
Integration of clinical, histological, immunohistochemical, and genetic information is needed to reach the correct diagnosis of cutaneous lymphoma. Nevertheless, despite the application of molecular biology techniques a percentage of lymphomas are not correctly diagnosed.
Due to the favourable prognosis of early stages the lack of curative therapy regimens for cutaneous lymphoma, multiple strategies have been proposed for the management of this condition. This includes topical corticosteroids, nitrogen mustard and carmustine, phototherapy, radiotherapy, biological response modifiers (bexarotene and interferon), chemotherapy (gemcitabine, pentostatin, etc), among others. Clinical type and particularly the clinical stage, toxicity profile as well as response rates should be taken into account in order to select the suitable regimen for each patient. Current concepts about classification, therapies and guidelines for the management of T-cell cutaneous lymphoma are discussed in detail in this paper.
