Septic pulmonary embolism as a manifestation of the severe sepsis in the puerperium. A case presentation

ABSTRACT
Introduction: septic pulmonary embolism is an infrequent syndrome. It is characterized by the embolization from an infection point, a pathogen formed thrombus and fibrin, to the pulmonary vascular system and its secondary infection.
Case presentation: a 20 year-old-female patient with severe acute endometritis after cesarean delivery, organ dysfunction and
Staphylococcus aureus bacteremia; and secondary pyodermitis to this bacterium. Fever, right pleuritic pain, dyspnea, cough and expectoration. The computerized axial tomography of the lung showed hyperdense, homogeneous image, and wedge-shaped form with sub pleural base in the middle lobe of the right lung and cavity images disseminated in both lung fields with predominance of the lower lobes, with regular internal and external contours, air therein; some confluents and other sub pleural located.
Discussion: septic pulmonary embolism is a serious but weird syndrome. It constitutes a diagnostic challenge for the wide range of clinical manifestations with which it is presented and the diverse etiologies that cause it.
Conclusions: The presence of bacteremia, fever and respiratory symptoms, with cavity lesions on computerized axial tomography of the lung contributed to the diagnosis of septic pulmonary embolism.
