Pronostic score of hospital nortality in patients with acute

ABSTRACT

Introduction: The knowledge of the severity of the heart failure allows to take therapeutic decisions, determine approaches of admission and discharge of the services of attention to the serious patient.
Objectives: Identifying the influence of different factors in the presage of the sick persons and compare the capacity to predict the state to the discharged patient of a new score with that of the classification of Killip and Kimball.
Method: observational study of cohorts in patients with acute heart failure in the unit of intermediate therapy of medicine of the Hospital "Miguel Enríquez" in three years of study. The sample was constituted by 256 patients.
Results: They presented statistical significance in their relationship with the state to the discharged patient the variables: acute coronary syndrome with heart failure, cardiogenic shock acute edema of the lung, antecedent of cerebrovascular illness and syncopate. The clinical form heart failure with high blood pressure didn't show a superior OR to 1 (OR= 0,86) in the unvaried analysis .The variables that showed independent and significant relationship with the state to the discharged patient were: the antecedent of cerebrovascular illness, syncopate; cardiogenic shock, acute edema of the lung, acute coronary syndrome and heart failure with high blood pressure, the hepatomegaly and the sex.
Conclusions: Most of the patients had more than 69 years, the mortality was significantly high. It was created a score of risk with a predictive capacity superior to the shown one by the prognostic classification of Killip and Kimball applied to the heart failure including different context to the heart attack.
