Effect of nifedipine coatcore and captopril on systolic diastolic and pulse pressure in patients with isolated

ABSTRACT
Background: Pulse pressure (PP) is an independent predictor of coronary heart disease. The aim of this study is to compare the effect of nifedipine coat-core
(NCC) with that of captopril on PP in elderly patients with isolated systolic hypertension (ISH).
Methods: After a 2 weeks wash-out, 30 elderly patients with ISH, self-measured their blood pressure (SMBP) for 3 days with a validate device; later on they received NCC 30 mg once a day for 2 months, when they
performed another SMBP in the same way. In the next stage, they received captopril 25 mg twice a day for two months, when they made another SMBP. Statistical analysis was done with ANOVA.
Results: Patients with NCC presented a 24 hours blood pressure reduction (178/88 to 134/86 mmHg, p < 0.0001). With
captopril blood pressure changed to 158/81 mmHg (p > 0.05). Patients reduced their PP with NCC by 53 ± 10 mmHg, whereas with captopril the PP reduction was
of 24 ± 12 mmHg.
Conclusion: Our results suggest that NCC is more effective than captopril for the control of PP in elderly patients with ISH. This may
be secondary to a greater reduction in systolic blood pressure with NCC,whereas captopril main decreases diastolic blood pressure in those patients.
