Correlation between clinical severity, TNF-α, and insulin-resistance in psoriasis

ABSTRACT
Background: Though the most visible component of psoriasis
is skin disease, recent evidence suggests that psoriasis is a systemic
disorder associated with multiple comorbidities, such as
insulin resistance syndrome and cardiovascular disease. Tumor
necrosis factor alpha (TNF-α), a cytokine associated to psoriasis,
induces insulin resistance through various mechanisms,
therefore it is probably one of the cytokines responsible for
the development of the mentioned comorbidities. The objective
of this study is to correlate: clinical severity, blood levels
of TNF-α, and the degree of insulin-resistance among psoriatic
patients who attended the Dermatology Clinic Complex during
December 2010.
Materials and methods: An analytical, correlational, randomized,
open trial with 23 patients with psoriasis and 8 patients
without psoriasis was performed. Weight, height, body mass
index, psoriasis area and severity index, blood glucose levels,
blood insulin levels and TNF-α blood levels were determined.
Statistical analysis was performed with EpiInfo and EpiDat.
Results: 56.52% of the psoriasis patients were female, 52.17%
were obese, 65.22% had mild disease, 17.39% were insulin-resistant,
and all had blood TNF-α concentrations below 31.25
pg/ml.
Conclusion: There was no correlation between disease severity
and the degree of insulin-resistance, blood glucose levels,
nor blood insulin levels among studied patients. There was no
correlation between the diagnosis of psoriasis and the presence
of insulin resistance, nor TNF-α blood levels above 31.25 pg/ml.
