Pitted keratolysis. Frequency and risk factors in firemen from Acapulco, Mexico, and literature review

ABSTRACT
Introduction: Pitted keratolysis, is a superficial, chronic and
asymptomatic infection of the cornea squamous layer of the
feet.
Objective: To know the frequency and risk factors of pitted
keratolysis in a group of firemen, and review the Mexican
literature.
Material and method: We performed an open, observational,
transversal, descriptive study, with firemen from Acapulco,
Guerrero in Mexico. A questionnaire and physical examination
study was performed in patients with pitted keratolysis.
Shave biopsy were obtained, (when patients consented) and
they were stained with hematoxylin and eosin, Gram and PAS.
Furthermore, a review of the Mexican literature from 1990 to
2011 was made.
Results: 25 firemen were included, 16 (64%) had pitted keratolysis,
50% was bilateral, located in metatarsal and heel. 10 (62.
5%) accepted the shave biopsy, with stains of H&E and Grampositive
bacterial structures; filaments and arthroconidias were
observed in 50% with PAS. Other dermatoses were found in
those 16 patients: Onychomycosis (5 cases),
tinea pedis (2 cases),
associated with onychomycosis (2 cases), tylosis (1 case). We
found only four publications of pediatric and adolescent cases
from 1990 to 2011, and no reports regarding adults.
Discussion and conclusions: In Mexico there are no published
studies of frequency or prevalence in adults. 69% of the
patients have wet feet, 56% reported hyperhidrosis and 31%
bromhidrosis. There are no studies that have measured the risk
of humidity.
