Comparison of effectiveness of dexamethasone vs diphenidol to reduce postoperative nausea and vomiting in patients undergoing tympanoplasty or mastoidectomy

ABSTRACT
Background: Postoperative nausea and vomiting are the most common
postoperative complications in otologic surgery. The antiemetic proper ties of dexamethasone and diphenidol have been described; however,
diphenidol is the drug currently used as first choice.
Objective: To compare the efficacy of dexamethasone and diphenidol
in adult patients undergoing tympanoplasty and/or mastoidectomy in
preventing postoperative nausea and vomiting.
Material and method: A randomized open clinical trial was done with 78
patients divided into two groups randomly. Group A received diphenidol
40 mg IV and group B was given dexamethasone 8 mg IV at the end
of the surgical procedure; the survey of Bellville for nausea and vomiting
was used in the immediate postoperative period and 8 hours later.
Results: Seventy-eight patients were enrolled (39 per group); 16 patients
(41%) in group A reported nausea (92.3%) versus 36 in group B (p=0.04).
Two patients (5.1%) in group A had at least one episode of vomiting
versus 12 (30.8%) in group B (p=0.003).
Conclusions: Dexamethasone is more effective than IV diphenidol to
reduce nausea and vomiting in adult patients undergoing mastoidectomy
and/or tympanoplasty.
