Value of patch testing in pediatric patients with atopic dermatitis: Experience at an urban healthcare center.

ABSTRACT
Background: Atopic dermatitis is a chronic multifactorial disease
most often seen in childhood, with frequent complications
including secondary bacterial or viral infections, and irritant and
allergic contact dermatitis.
Material and Methods: This was a prospective, descriptive
study of pediatric patients aged 6 months to 18 years, previously
diagnosed with atopic dermatitis and suspected of contact
dermatitis. Patch tests were applied in standard IQ
® pediatric
chambers, using Hanifin and Rajka criteria to diagnose atopic
dermatitis and SCORAD for severity rating. Incidence and simple
average were determined with the SPSS X statistic package.
Results: The sample consisted of 21 patients of both genders,
aged 6 months to 18 years, with a history of atopic dermatitis,
rhinitis, and asthma. Skin lesions showed the following patterns:
flexural (13 cases), facial (1), nummular dermatitis (3) and juvenile
plantar dermatitis (4). The severity was rated mild to moderate
(SCORAD 23.8).
Patch tests included 23 reagents and resulted in 33.3% positive
outcomes (7 cases sensitive to nickel; 1 sensitive to chromium)
with 66.6% negative responses.
Discussion: Patch testing to ascertain contact dermatitis in
patients with atopic dermatitis produced a very low percentage
of positive results in our study. To establish the true incidence
and prevalence of this association, it is necessary to conduct a
similar trial with a larger population sample.
