Impact of the quality assurance program for mammograms in Colombia

ABSTRACT

Background: in Colombia breast cancer in the female population has the highest rates of incidence and mortality of all types of cancer, with an upward trend. The Ministry of Health and Social Protection, through the National Cancer Institute (Spanish acronym INC), has considered it necessary to strengthen mammogram-based early detection programs by implementing quality assurance programs in the nation’s mammogram services.
Objective: present the results of follow-up evaluations of the mammogram quality control program in a sample of six cities in Colombia.
Materials and methods: in six Colombian cities we conducted an evaluation of mammogram quality control in a sample of 28 radiology centers which offered the service and which participated voluntarily. The study was divided in two stages: in the first we conducted an initial visit, based on which we issued a series of recommendations to improve the quality of service. In the second stage we conducted follow-up visits to the same centers to evaluate the improvements implemented and the impact of the quality assurance program.
Results and discussion: we compared the results of our mammogram quality control measurements at the follow-up visits at 28 centers in Colombia with those from the initial visits. The results showed a significant improvement in mammogram image quality, in infrastructure conditions, in calibration of mammogram equipment, and in image viewing conditions. However, we still observed significant deficiencies in some of the parameters evaluated.
Conclusions: application of the quality control protocol at different mammogram centers allowed us to measure the program’s impact in Colombia. For that reason, we have confirmed the need to implement quality assurance programs that raise the effectiveness of different mammogram services in the country, and by extension of the mammogram screening program being established in Colombia.
