Differential features of ischemic and hemorrhagic stroke in–hospital mortality

ABSTRACT
Objective: To determine clinical and health care services differential features related with ischemic and hemorrhagic stroke mortality.
Methods: An observational explicative study of clinical and health care services variables was carried out in patients deceased with diagnosis of stroke in Dr. Ernesto Guevara de la Serna Hospital during 2012 year.
Results: Study population included 96 patients deceased with stroke (54 ischemic and 42 due to cerebral or subarachnoid hemorrhage). Ischemic stroke age was over 40 years (median of 72.03 years). Hemorrhagic stroke was included in every age group (median of 60.21 years) and arterial hypertension was more frequent. Ischemic stroke have more left ventricular hypertrophy, chronic ischemic cardiac disease, diabetes mellitus, previous ischemic cerebrovascular disease, auricular fibrillations and chronic peripheral arterial insufficiency. Initial stroke unit admission included only 25 % of deceased patients. More patients with hemorrhagic stroke received mechanical ventilation and intensive care unit admission. Several findings increase the diagnostic probability of hemorrhagic stroke: vomits in onset, acute headache, stupor or coma, hypertensive crisis, and Siriraj stroke scale › 1. Probability of hemorrhage was minor in cases with diabetes mellitus, chronic ischemic cardiac disease, chronic peripheral arterial insufficiency, and previous ischemic cerebrovascular disease.
Conclusions: Some clinical and health care services differential features were related with ischemic or hemorrhagic stroke. More emphasis is need regarding investigations about health care services practices, comorbidities control and complications in deceased patients with stroke.
