Quality of life in patients with vascular dementia post–ischemic stroke (cohort study)

ABSTRACT
Objective: To establish factors related with quality of life in patients after a first acute ischemic stroke, with or without vascular dementia (VD).
Methods: We performed a cohort prospective study from January 2011 to June 2013 in patients with clinical and tomography criteria´s of first ischemic stroke admitted in rooms of Internal Medicine Service of the "Iván Portuondo" and "Ciro Garcia" Hospitals (Artemisa Province, Cuba). For sampling of conglomerates, using the criteria’s diagnostic of DV, they were selected at the expenditure hospital for 2 groups of 90 patients: Group A (Stroke and VD) and Group B (Stroke without VD). This sample was followed in regional consultation of cognitive impairment by control period of 18.5 &3177; 3.2 months (range: 0.03–24.2). Quality of life like was selected as dependent variable and as independent variables: functional recovery, state of spirit, comorbidity, age, gender, civil state and recurrence of stroke.
Results: 64.4 % of patient of the group A had bad quality of life being the age bigger than 75 years (p=0.016) and the
associated comorbidity the variables of more significance statistical. Hypertension was detected in 75.5% of the total patient
studied. Group A included 51.7 % of patients with bad quality of life and uncontrolled hypertension (Relative risk 10; IC 7.8–
14.3). The Index of global Charlson (18 ±3.4) was the variable more valuable in prognostic analysis (Exponential value of B
coefficient 70.123, p=0.011).
Conclusions: The coexistence of VD in the patients with a first stroke associates with bad quality of life in relatively short
time period very related with the poor control of the risk vascular factors.
