Exacerbation of cerebral edema associated with the use of Gelfoamä after resection of meningioma

ABSTRACT
Adequate hemostasis in neurosurgery is of paramount importance in a
daily practice. There is a variety of hemostatic agents (Gelfoam
TM, Avatine
TM,
Surgicel
TM) which have demonstrated to be safe and biocompatible.
Inflammatory response have been described against hemostatic agents
that increase the peritumoral brain edema (PBE) after surgery in a few
cases. We report the case of a 54-year-old male with frontal meningioma
and during surgery, Gelfoamä was used as hemostatic agent; despite he
was previously and after surgery treated with steroids, three days after
tumoral resection PBE was increased. We reviewed the literature looking
for possible pathophysiological mechanism, prevention and treatment.
