Factors affecting postoperative bleeding in cardiac surgery with extra-corporeal circulation

ABSTRACT
Cardiac surgery patients are exposed to foreign materials from the use of the extracorporeal circulation, the oxigenator, and the tubes from the circuit, all of which produce different physiopathologic changes in the hematologic system, which added to preoperative abnormalities of the coagulation system that commonly are present in these patients, lead to severe coagulopathies and excessive bleeding which must be appropriately treated in the pre, trans and postoperative period. We review the main hematologic abnormalities produced by the use of extracorporeal circulation in response to blood contact with non-endothelial surfaces, hemodilution y other coagulation factors unrelated to the extracorporeal circulation, as well as the clinical measures to be adopted to prevent or treat excessive bleeding in these patients.
