Hemodialysis effect on ventricular function

ABSTRACT
Introduction: Chronic Kidney Failure has an estimated incidence of
377 cases per million persons, with about 52,000 patients in renal replacement
therapy. Chronic Kidney Failure is among the first ten general
mortality cause, is the eighth cause of death in males of reproductive
age and sixth in adult woman. The precise effects of hemodialysis on
the ventricular function of patients with chronic kidney disease are
unknown. In the present study we compare the echocardiographic
changes observed in patients undergoing peritoneal dialysis with does
undergoing hemodialysis.
Material and methods: Prospective, longitudinal, cohorts study. Patients
from the nephrology service at Dr. Dario Fernandez Fierro Hospital were recruited. They were placed in one of two groups, those in a hemodialysis
program (group 1) and those in peritoneal dialysis program
(group 2). An echocardiogram was done to each patient in each group
at the start of the protocol and a second one at three months’ time,
measuring the Dp/Dt.
Results: A Dp/Dt increased in 30%, decreasing in 0% and had no change
in 70% of patients in peritoneal dialysis. In the hemodialysis group the
Dp/Dt increased in 11.1%, decreased in 55.6% and had no change in
33.3% of patients (
p ‹ 0.05).
Discussion: This study shows that significant difference exists on ventricular
function measured by Dp/Dt after three months in patients
treated with hemodialysis compared with peritoneal dialysis.
