ABSTRACT
Introduction: Related to rapidly progressive but potentially reversible encephalopathy, we have autoimmune dementias and encephalopathy. Antibodies against the N–methyl–D–aspartate (NMDA) receptor have been associated with a form of subacute encephalitis, more common in children and adolescents.
Case report: A 9 years old girl, with unremarkable past medical history (perinatal or postnatal), with a normal psychomotor development, seeks for medical attention in the emergency room. She has a history of complex focal seizures with secondary generalization. Also abnormal gait, mutism and behavior disturbances at home, along with sleep disorders and psychomotor agitation were reported. Infectious and metabolic causes were excluded. Studies were positive for antibodies against the NMDA receptor, as part of non–neoplasm related auto–immune encephalitis.
Conclusions: Regarding differential diagnosis for subacute encephalopathy in young people, it is important to rule out autoimmune disorders. Anti–NMDA encephalitis leads to a very important diagnostic, because its relative reversible condition with treatment in a reasonable time.
