Periodontally accelerated orthodontic treatment: a comparison of surgical techniques

ABSTRACT
Corticotomy dramatically reduced the treatment time without any adverse effects on the periodontium and the vitality of the teeth. This technique has several advantages, including faster tooth movement, shorter treatment time, enhanced post-orthodontic treatment stability, and safer expansion of constricted arches. One of the minimally invasive techniques is with a piezosurgical microsaw to eliminate cortical bone resistance. Elimination of flap results in the reduction of surgery time, and minimum discomfort to the patient. The aim of this study was to compare the vestibular corticotomy versus vestibular/palatal corticotomy to establish a difference between both procedures in reference to dental movement and patient discomfort. A total of 11 patients were included in the study: vestibular alveolar decortication alone (VADA) versus combined decortication (VADAC). Surgery was performed around the buccal or buccal/palatal aspects of the maxillary canines. Measurements were done at baseline (day 0: no treatment rendered) and every 2 weeks posterior of activation. The results showed statistically significant increased tooth movement in the side with combined decortication compared with VADA. In both procedures the patient discomfort was not statistically significant.
