ABSTRACT
Objective: To evaluate the results of percutaneous angioplasty for critical lower limb ischemia by determining
the percentage of limb salvage, wound healing and efficiency in controlling pain.
Material and methods: We perfomed a prospective analytical study that included all patients who
underwent percutaneous infrapopliteal angioplasty from March 2009 to June 2011 in the Department
isqueof
Angiology, Vascular and Endovascular Surgery, at the Centro Médico Nacional 20 de Noviembre,
ISSSTE. Twenty-three patients were evaluated to the study. Sixteen patients were included and 7 eliminated
because an angioplasty was perfomed in a proximal arterial segment. It was assessed the pain
intensity pre-treatment, with the visual analogue scale (VAS). Angiographic findings were classified according
to Jenali’s Classification, and followed the intervention protocol established. Evaluation was
permed at a week, 3 an 6 months post procedure evaluating pain intensity with VAS, wound condition
and limb salvage.
Results: Minor amputations were done in 3 patients during the same surgical procedure and 6 more
during follow-up. At 6 months 75% (n = 12) of patients had a proper evolution of the wound. A major
amputation was performed at three months due to critical ischemia, with limb salvage in 94% of patients
at 6 months. Another important outcome was the diminishment of pain. From a 7-10 VAS to a 0-
2 at six months (p = 0.045).
Conclusions: The percutaneous infrapopliteal angioplasty is a minimally invasive and effective way
to reduce pain, and improve quality of life, promote healing of a wound or limb salvage.
