Primary tonsillar tuberculosis in a patient with rheumatoid arthritis treated with anti-TNFα (etanercept)

ABSTRACT
Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. Pulmonary affection accounts for its main clinical presentation. Immunocompromised patients present a higher incidence of the disease in which extrapulmonary manifestations are more common. In this case report we present a 53-year-old female patient with active rheumatoid arthritis under etanercept (α tumor necrosis factor inhibitor) therapy that develops the unusual presentation of primary tonsillar tuberculosis infection with secondary vertebral osteomyelitis. Diagnosis is confirmed by biopsy and polymerase chain reaction. She is currently received tuberculosis therapy consisting of isoniazid, rifampin, pyrazinamide and ethambutol.
