Neck pain in the patient with rheumatoid arthritis

ABSTRACT

Antecedents: Pain in the cervical column is a frequent complain and is constituted by a heterogeneous group of diseases. It’s been estimated that its prevalence in general population of industrialized countries is of 20% and that is more frequent in women. Objective: To present to the reader actual information about the mechanisms involved in the genesis of this painful syndrome in patients with rheumatoid arthritis. Results: Rheumatoid arthritis is a disease associated to pain in cervical spine; it promotes alterations in skeletal axis in 10 to 86% of patients. The instability of this skeletal segment is potentially lethal and cause biomechanical alterations capable of generate neurological symptoms, such as: (i) radiculopathy, (ii) mielopathy, and (iii) involvement of cranial nerves. Diverse therapeutic approaches had been used to relief pain, such as: (i) pharmacologic, (ii) non-pharmacologic, (iii) interventionist, and (iv) surgical. Conclusions: Pain in the cervical spine requires a precise evaluation that allows to identify its etiology and in consequence provide an optimal therapeutic approach.
