ABSTRACT
Introduction: Spontaneous intracerebral hemorrhage (SICH) represents one of the most severe subtypes of stroke.
However, and despite a high incidence, medical treatment is almost limited to life support and to control intracranial
hypertension and indications of surgical treatment are poorly defined.
Case reports: we are reporting two cases operated on in our hospital with lobar SICH in which the clots were evacuated through a cortical puncture and aspiration guided by transcortical echography.
Conclusions: This is a surgical procedure that has been used in the world but in our country is unusual for SICH evacuation and all of our hospitals have the technological devices required.
