Diabetes mellitus and stroke risk

ABSTRACT
Objective: To review the available evidence those relate diabetes with the risk of stroke, and the role of glycaemic control according to some strategies in risk reduction of stroke.
Development: Diabetes is associated with relative risk of ischemic stroke between 1.8 and 6, but for hemorrhagic stroke the association is controversial. Women with diabetes type 1 have more risk than those with diabetes type 2. Moreover, diabetes predict extend hospital stay and greater mortality and incapacity in stroke patients and patients who die due stroke have higher levels of HbA1c and glycaemia. Besides, almost 9% of stroke recurrences were attribute to diabetes, because of that diabetes was included in prognosis scales of stroke recurrences. Even though, intensive treatment for glycaemia levels in diabetes type 1 patients was associate with lower levels of HbA1c and microalbuminuria and risk reduction of cardiovascular
events in 42%, big trials about intensive treatment for type 2 diabetic patients reduce microvascular complications but not major cardiovascular events. Nevertheless, strict control of blood pressure, use of “statins”, and type DASH diet lead to an important reduction of stroke risk in patients with diabetes type 2.
