Effectiveness of Preoperative Glossopharyngeal Block in the Control of Postoperative Pain in Subcapsular Tonsillectomy

ABSTRACT
Background: So far there is not a technique or technology associated
to tonsillectomy that may help to perform it without pain.
Objective: To evaluate the effects of preoperative peritonsillar injection
of lidocaine plus adrenaline 1:200,000, clonidine and fentanyl in
children undergoing tonsillectomy/adenotonsillectomy
versus placebo.
Patients and method: A prospective, comparative study was done with
96 children (age 2-14 years) submitted to tonsillectomy-adenotonsillectomy,
divided into two groups: in one a preoperative block was done at
tonsillar fossa with lidocaine 2% with epinephrine 1:200,000, fentanyl
and clonidine, and in the other group placebo was administered. Visual
analog scale was used for postoperative pain assessment; follow up after
surgery was of seven days.
Results: Median visual analog scale for the infiltration group was significantly
lower in days 1 through 4 (<0.001); after day 5 no significant
differences were observed, all patients were outpatients.
Conclusion: Preincisional injection of lidocaine plus adrenaline
1:200,000, fentanyl and clonidine decreases significantly postoperative
pain and improves soft diet in the first 48 hours without a serious risk
of a severe complication.
