ABSTRACT
Introduction. Down syndrome is a congenital disorder associated with abnormalities in multiple organ systems that offer challenging during the anesthetic management. Objective. To present the considerations to have in mind when choose epidural anesthesia technique. Material and methods. Female patient in the fifth decade of life programmed for umbilicoplasty as an elective ambulatory surgery and managed under lumbar epidural anesthesia. The medical record includes predictors of difficult airway, physical characteristics of Down syndrome, surgical history managed under local anesthesia, ventricular septal defect in childhood, and sulfa allergy. Anesthesic management is decided with caudal epidural catheter and administration of fractional doses of lidocaine 2% and lidocaine with epinephrine achieving satisfactory analgesia. Results. The choice of epidural anesthetic technique, in this patient, together with specific preventive in the perioperative period allowed to provide quality care in the surgical setting for this patient. Conclusions. We consider strategies for the anesthetic plan to promote quality care and warmth of patients and families affected by this chromosome disorder. Multidisciplinary management brings to fruition the surgical experience of patients with Down syndrome.REFERENCES
