ABSTRACT
Pigmented purpuric eruptions comprise a group of benign that are characterized clinically by pinpoint petechiae and purpura on a hyperpigmented base and histologically by capillaritis. The etiology of this type of disorders is unknown, although aberrant cell-mediated immunity has been proposed. We present a 28-year-old woman with purpuric and hyperpigmented macules on the lower extremities of 1 year’s duration. The patient was otherwise healthy and laboratory studies were normal. The histologic examination revealed perivascular lymphocytic infiltrate in the superficial dermis and focal extravasation of red blood cells. The clinical and histologic findings were consistent with pigmented purpuric dermatitis. She was treated with topical steroid once daily. Follow-up evaluation at 8 weeks that macules had largely cleared.
