ABSTRACT
Introduction: Laparoscopic appendectomy (LA) in principle in obese children was contraindicated; it is commonly used for the treatment of appendicitis.
Objective: To compare laparoscopic versus open appendectomy in obese children.
Material and methods: A retrospective study between 2009 and 2012 was performed. Obese was defined as those with more than two standard deviations above the mean standardized weight of his age,
determined by body mass index (BMI) 30 kg/m
2. 61 of 238 (25.6%) patients had open appendectomy and 177 (74.3%) underwent laparoscopic appendectomy. The length of hospital stay, operative time, complication rate and frequency of taking analgesics for postoperative pain relief were compared between open and laparoscopic appendectomy in children with obesity.
Results: Laparoscopic appendectomy for acute appendicitis in obese children is associated with less operative time, less presentation of complications (46.8 versus 59.87 min, p ‹ 0.05) (5 versus 8.2 %, p ‹ 0.05) and less need for postoperative analgesia (6 979 versus 4 739, p ‹ 0.05).
Conclusion: Laparoscopic appendectomy should
be the procedure of choice for treatment of acute appendicitis in obese children.
