PROMETHEUS as a bridge to liver transplantation in patient with acute-on-chronic liver failure: A case report

ABSTRACT
Acute-on-chronic liver failure, a syndrome precipitated by acute liver injury in patients with advanced cirrhosis, is
associated with multiorgan dysfunction and high rates of mortality. Liver support systems have been developed in an
attempt to improve survival of patients with acute-on-chronic live failure by providing a bridge until recovery of the
native liver function or until transplantation. We present a case of a 47 year-old male with diagnosis of acute-onchronic
liver failure secondary to mixed damage (alcohol plus acetaminophen). Within the first hours in our center, he
developed significant neurological impairment, renal failure and seizures, with no response to medical treatment.
Therefore, we decided to use extracorporeal liver support system (PROMETHEUS
®, Fresenius Medical Care AG, Bad Homburg, Germany ). The patient received four sessions prior to liver transplantation.
