Conus medullary lipoma in a child with Klippel-Feil syndrome. Case report

ABSTRACT
Lipomas of the conus medullaris and terminal filum are the most common form of occult spinal dysraphism. Central nervous system manifestations associated with the Klippel-Feil syndrome are not uncommon, and are probably caused by a congenital myelodysplasia. A case of an 11-year-old boy with Klippel-Feil syndrome who presented with back and radicular pain and paresthesia in both lower extremities is reported. Lumbar magnetic resonance imaging revealed the presence of an intramedullary lipoma at the conus medullaris. L2-L3 laminotomies with intraoperative somatosensory evoked potentials were performed to remove the lesion, and a macroscopic and histopathological analysis to confirm the diagnosis. With one year of follow-up, the boy showed no neurological deficit. After the review of the English literature, we found just one case of spinal lipoma associated with Klippel-Feil syndrome in an adult, so this could be the first case reported in children.REFERENCES
