Moyamoya disease in a patient with Down syndrome

ABSTRACT
Moyamoya disease is an idiopathic arteriopathy which causes a progressive narrowing of Willis polygon and the internal carotid artery. This syndrome is secondary to different entities and manifests as transitory ischemic strokes, headaches, infarcts or cerebral hemorrhage. We present the case of a male patient age 25 years, with Down syndrome, who was admitted to emergencies with left hemiparesis. Angiotomography showed an occlusion of the communicating segment of the right internal carotid artery and stenosis of the supraophthalmic segment of the left internal carotid artery. A vascular network formed by small blood vessels was observed, which compensates for the irrigation defect on the right side. Magnetic resonance showed findings compatible with a subacute ischemic lesion in the region of the right medial cerebral artery. At admission the patient was stable, showed gradual improvement, and was released. The surgical option was discarded.REFERENCES
