Risk Factors Associated with Gastropathies. Trinidad, Sancti Spiritus

ABSTRACT
Background: it is important to identify factors increasing the
risk of serious gastrointestinal complications and to determine
the actions to take in order to reduce them.
Objective: to identify risk factors associated with gastropathies
in patients attending the gastroenterology consultation.
Methods: a study including cases and witness selected out of
patients attending the gastroenterology consultation was
conducted in a polyclinic in Trinidad, Sancti Spiritus in the year
2008. 104 cases of gastropathies diagnosed by endoscopy and
208 witnesses were studied. The variables analyzed were: age,
sex, smoking habit, coffee intake, alcohol consumption and
non-steroidal anti-inflammatory drugs. Dichotomous logistic
regression was used to control de confounding bias, and
subsequently the population attributable risk percent and the
dose-response curve were calculated.
Results: the independent risk factors for gastropathies
identified were: ingestion of non-steroidal anti-inflammatory
drugs (OR 4,50 IC; 2,60 to 7,80 and RAP % 56,75 IC; 37,50 to
71,83) with a significant dose-response curve for daily tablets
and months of consumption; smoking habit (OR 2,82 IC; 1,65 to
4,84 and RAP % 38,97 % IC; 18,57 to 57,40) with a significant
dose-response curve for daily cigarettes and years of
consumption. Non-steroidal anti-inflammatory drugs identified
as independent risks were indomethacin, ibuprofen and aspirin.
The increase in 1 year of age also represented a risk in this
series.
Conclusions: smoking habit in addition to the ingestion of
indomethacin, ibuprofen and aspirin were independent risks for
gastropathy. Physicians should take into account the effects of
these medications to avoid prescribing high doses of them for
long periods.
