Postoperative analgesia with nalbuphine

ABSTRACT
Objective: To asses the post-surgical analgesic efficacy of epidural nalbuphine without parabens after cesarean section.
Materials and methods: An observational, transversal, prospective, simple descriptive study was carried out in 60 patients with full-term pregnancy undergoing cesarean section, ASA 1. The level of blockage was L2- L3, both for surgery and for post-surgical analgesia. All patients were given 10 mg nalbuphine without parabens (1 ml), diluted in 9 ml normal saline, for a total of 10 ml, at the start of post-cesarean pain, and an identical second dose 8 hours later. Patients were monitored at 30 min, 1, 2, 6, and 8 hours after the first and second doses. Pain was measured using an visual analogue scale (VAS) ranging from 0 (no pain) to 10 (intense pain). Sedation was evaluated using the Ramsay sedation scale, ranging from 0 (none, patient awake), to 4 (very severe, patient asleep with respiratory depression). Monitoring included heart rate, blood pressure, mean blood pressure, oxygen saturation by pulse oximetry, as well as side effects (nausea, vomiting, itching, urinary retention) and the number of doses of rescue medications.
Results: Epidural nalbuphine without parabens (1 ml diluted in 9 ml normal saline) was shown to offer good postoperative analgesia lasting 8 hours, with little or no side effects such as sedation, nausea, vomiting, pruritus or urinary retention. No serious disturbances were seen in heart rate, blood pressure, medium blood pressure, and oxygen saturation, and the need for rescue medication was minimal.
