Subclinical thyroid Dysfunction in Adults Evaluated at the Laboratory of a Third Level Unit

ABSTRACT
Background: Subclinical thyroid dysfunction is more common
than overt dysfunction; however, its prevalence is significantly
different from one region to another, even in the same country.
Objective: To determine the prevalence of subclinical thyroid
dysfunction in evaluated adults in the clinical laboratory of a
tertiary care unit.
Material and method: A descriptive and retrospective study
was done including data from individuals aged 18 years or older,
assessed by measuring thyrotropic hormone (TSH) and free thyroxine
(fT4), from February to June 2011, in the clinical laboratory
of Regional Hospital Valentin Gómez Farías, Jalisco, Mexico.
Data were classified according to functional status, gender, and
age groups (younger and older adults). Statistical analysis was
performed with the χ
2 test.
Results: Data from 2,091 individuals were analyzed: 506 old
adults (68.59 ± 7.89 years) and 1,585 young adults (44.15 ± 9.84
years). Prevalence of subclinical hypothyroidism was 8.8%: men
7.1%, women 9.2% (
p = 0.173); old adults 11.1%, young adults
8% (
p = 0.034). Prevalence of subclinical hyperthyroidism was
11.1%: women 12.7%, men 5% (
p ‹ 0.001); young adults 10.6%,
old adults 12.6% (
p = 0.201).
Conclusions: Subclinical thyroid dysfunction was found in one
fifth of evaluated individuals and it is more frequent than overt
dysfunction, about 4:1. It is advisable to conduct a study with a
representative sample to generalize results.
