ABSTRACT
A precise diagnosis should be based on an objective and effective methodology, which should begin with the elaboration of a) topographic diagnosis, check out a b) differential syndromatic to reach a c) etiological diagnosis, without forgetting that medicine is not an exact science, by what we must never ignore that we have to make a last d) differential diagnosis. The pathology of the column cannot be the exception to this rule, so it should remember, have a critical and ordered thinking. This can be done using a diagnostic strategy in which is called «vertebral bone destruction syndrome». This is defined as a pathology of multiple etiology, characterized by changes in the anatomy of the spine, specifically with deformity and surrounding volume in one or more vertebral bodies, is accompanied by pain and functional disability by mechanical and neurological alterations, considering as possible causes, to the infectious, tumors
and metabolic pathologies. These pathologies has common characteristics among themselves; however, for the identification of the syndrome it is necessary to consider the singularities that these morbidities share, in order to carry out a process that enables the systematization in addressing; this way could optimize the time between the identification of the pathology and the initiation of treatment with, in addition to the detection of more sensitive and specific for each nosological entity studies knowing its positive predictive value.
