Clinical aptitude of nurses at internal medicine and surgical services

ABSTRACT
Introduction: The type of education in which nurses are educated and the characteristics of their work environment have contributed to play a role subordinated to doctors. As a consequence their workload tends to be more dull than analytic.
Objective: To find out the level of development on clinical apti-tude during the nursing care in internal medicine and surgical services’ patients.
Methodology: A transversal, comparative study was done including the total of nursing assistants and professional nurses who worked at internal medicine and surgical services, at two hospitals of second level of care on Veracruz. To evaluate the clinical aptitude during the nursing care, it was designed an instrument (IC=0.90) with ten real clinical cases through three indicators: recognizing signs and symptoms, integration of diagnosis and nursing activities.
Results: It was found that 72.5% from the total of nurses have developed certain level of clinical aptitude even though the most of them (65%) were placed in an incipient level. It was not found correlation between indicators; the score in nursing activities was significant lower than the others indicators (p=0.01). There were not differences when comparing facilities and time of working. The median of obtained scores from professional nurses was of 37, while from nursing assistants was of 29.5 (p‹0.05).
Discussion: The obtained score on nursing actions has to do with the nurse’ theoretical support regarding her or his daily work; this means that the results support the following hypothesis: that type of work seems to be characterized by daily exertion, more than by well supported clinical reasoning; fact that coincide with results found in previous studies.
