Detection of cerebral circulatory arrest by transcranial Doppler. Implications as a useful method for the determination of brain death

ABSTRACT
The clinical diagnosis of brain death (BD) has never been easy for most clinicians. This has led to the need for studies to confirm the diagnosis of BD. Currently, we have the electroencephalogram, angiography, which corroborate the clinical diagnosis, however, sometimes the effect of drugs or complicated technology, represent difficulties to confirm the diagnosis of BD. The transcranial Doppler ultrasonography is an alternative efficient, safe, and noninvasive method, performed besides the bed of the patient, and allows real-time knowledge of the hemodynamic status of intracranial vasculature. Through an adequate analysis of hemodynamic spectra, it is possible to establish the diagnosis of cerebral circulatory arrest. This review examines the characteristic patterns of cerebral circulatory arrest.
