Episodic meningitis as a manifestation of complex spinal dysraphia

ABSTRACT
Introduction: The spinal dysraphims form a heterogenous group of anomalies of the neuroectodermal weave, caused by a failure in the disjunction of the neural tube during the period of embryogenesis. They are possible to be classified in open or hidden in dependency of the presence of visible neural elements or no. Their clinical manifestations are variable, affecting the skin, bony structures, urologic and nervous system. A feminine patient of seven years with complex spinal dysgraphia appears and little she frequents that she was pronounced by episodes of meningitis to repetition. Its diagnosis is realised through the RMN, essential technique in these pathologies. Clinical case: Student of seven years of age that enters with meningitis diagnosis, with antecedents of two similar previous episodes in a period of five years, with good antimicrobial answer. To the physical examination lumbo-sacrum is demonstrated to the presence of a thermal sine and after realising studies of diagnostic neurograph a bifid thorn of several lumbar segments, marrow anchored below dermoides L4 and a lumbar cyst intrarachidian under previous location in the channel. To the patient lumbar descompressive laminectomy is realised to him, extirpates the sinus passage, the medullary anchorage is freed and dermoides exeresis of the cyst are realised. Conclusions: Before episodes of meningitis to repetition one of the clinical elements to evaluate it is the possibility of the existence of malformations that can serve as front door of pathogenic agents the SNC, as much in cranial regions as spinal, as it happened in our case. The presence of associated bifid thorn to dermatologic, orthopaedic, urologic and neurological manifestations causes that the possibility of existence of a dysraphism exists, is due to realise the surgery of opportune form and so it is avoided complications and it is guaranteed the neurological reversibility of the symptoms and signs.REFERENCES
