Stroke as a complication of atrial myxoma. Report of six cases and review of literature

ABSTRACT
Introduction and objective: Although the most common neurological manifestation of atrial myxoma (AM) is the embolic stroke, there are some nonspecific systemic symptoms and minor embolic events that are often the first manifestation and in the absence of other cardiac symptoms go unnoticed. The aim of this paper is to present the experience of 6 cases of atrial myxoma with systemic and neurological manifestations.
Patients: We describe the clinical and radiological manifestations of 6 consecutive patients with AM treated between 1995 and 2008 as well as its evolution and prognosis. The average age of patients was 34.1 years (range 16 to 42 years), 5 were women, and 3 cases were initial systemic manifestations, including phlogosis, fever, fatigue and arthralgia. In 5 cases the neurological manifestation was cerebral infarction and in 1 lobar hemorrhage. In 4 cases, brain MRI showed multiple bilateral ischemic lesions in different arterial territories. The diagnosis of MA was made at a mean of 9.6 months (6 to 12 months) of evolution. The functional outcome was good in all cases.
Conclusions: Patients with neurological manifestations of atrial myxoma are younger patients, and the initial manifestations may be nonspecific. In our series the presence of atrial myxoma was more common in women and the most common manifestation was a cerebral infarct.
