Brain Attack Surveillance in a Mexican Communi ty: Des ign of a populat ion-base s tudy of cerebrovasculares diseases in Mexico

ABSTRACT
Population-based research in Mexico is lacking. Data based on government health surveys or single hospital reports are subject to significant biases. The current proposal is directed to establish a rigorous populationbased stroke research in a Mexican community. The main aim of this project is to perform rigorous population-based stroke surveillance in a representative Mexican community (Durango Municipality, Mexico). This project emerged from an international collaboration that attempts to study stroke epidemiology in a similar fashion to BASIC study (“Brain Attack Surveillance in Corpus Christi”) in Texas, USA, and then the current proposal is called “Brain Attack Surveillance in Durango” (BASID) project. In this study we describe the designing of the project including selection of the community, the process of staff training as well as the process of stroke validation. For the first time, this project has provided estimates about incidence rate and case fatality in hospital rate of stroke in a Mexican population. The crude annual incidence for total (first-ever and recurrent strokes) was 143 per 100 000 persons per year (95% CI, 128 to 158), without sex differences. In-hospital case fatality stroke rate was 34% but was as high as 50% in hemorrhagic strokes. BASID is now poised to serve as a population for studies of stroke epidemiology in order to identify specific targets for evaluation of public health interventions to reduce the stroke burden in Mexico.
