Senile burns patients. Comparative study

ABSTRACT
We studied 2,562 records of burned patients treated at the Burns Unit of the «Dr. Victorio de la Fuente Narvaez» Traumatology Hospital, of the Mexican Institute of Social Security from January 2000 to December 2011. An analytical study was performed to determine risk factors in the prognosis and outcome. We compared the following groups: senile and non-senile patients, octogenarian and those who were not octogenarian patients, by age group and time period in which they were treated. 10.2% were senile, with a burned area of 17.5%. 45.5% presented co morbidity and functional limitation in 19.4%. Complications for the senile patients were 20.2% and for non-senile of 12.8% (p = 0.001; OR 2.39) and mortality for the senile was 10.7%, and 6.3% for non-senile (p = 0.008; OR 2.7). Octogenarian patients had higher mortality than non-octogenarians (21.3 versus 13.1%; p = 0.063, OR 3.4) and complications (32 versus 22.5%; p = 0.068, OR 2.86), there were no significant differences with other variables. There were higher mortality and complications, the older the patients and the greater the extension. Patients with score of seven or less had a survival of 98.5% and those who had 12 or more were 61.5%. Mortality increased 18.1% for each 10% of burned body surface (R 0.921, p ‹ 0.001) and 5.3 percent per decade of age (R 0.894; p = 0.013). When comparing results of the 2000-2002 period with the 2009-2011 period, we found a decrease in the mortality of 10.2% (p = 0.012; OR 8.76) attributable to more resources, new technologies and more experienced staff.
