Current state of the hormonal replacement therapy with dehydroepiandrosterone

ABSTRACT
Background: According to the WHO, aging is one of the greatest global public health challenges. On any medical approach, from prevention to therapy, physiopathological understanding of disease mechanisms is necessary not only to achieve success, but also, to avoid undesired complications that in the case of aging represent a broad spectrum of chronic diseases that may condition catastrophic costs in terms of treatment and rehabilitation.
Goal: To establish the state of the art of DHEA’s role on aging process including the diagnostic and therapeutical perspectives.
Methodology: This is a bibliographic review of relevant national and international medical publications.
Results: DHEA is an adrenal, cerebral and gonadal steroid derived from cholesterol and precursor of sexual hormones. Plasma and serum levels of DHEA-S have been extensively used on Cushing Syndrome, Congenital Adrenal Hyperplasia And Adrenal Tumors Including Virilization, Hirsutism, Polycystic Ovarian Syndrome and several Psychiatric Problems Including Depression A novel application for this test is chronobiological marker for aging evaluation. For this purpose it is convenient to consider gender and age measured as decade in order to compare obtained results with pre-established chronobiological reference limits.
Conclusions: There are many not-answered questions and doubts surrounding DHEA. Even when it seems clear that it is playing an important role on aging process it is difficult to assure that nutritional supplementation on low doses or pharmacological intake on high doses will have a positive impact on longevity that depends on bio-psycho-social harmony and psycho-neuro-immune-endocrine (PNIE) homeostasis that we are just recently starting to understand. Even though that DHEA is being used extensively as an ant aging nutraceutical we still need a solid, scientific, prospective, long-range protocol in order to define the precise role and capacity to extend longevity and to improve the quality of human life.
