Multidisciplinary approach of the diabetic foot

ABSTRACT
The diabetic foot (DF) is a complication of diabetes mellitus, and those who suffer it have 30 more times probabilities to result in amputation. This complication is characterized by neuroischemic manifestations, infection and foot deformities. The beginning of the clinical alteration of DF is due to a loss of metabolic control, as well as in alterations in several mechanisms, such as neuropathic, microvascular and macrovascular in which the atheroma plaque is built, following a progression and evolution of neurovascular complications. The most accepted classification for this illness is the one of Wagner. The physiopathologic process of DF must be handled in an integral form, because all of these occur simultaneously, and should be handled in a multidisciplinary approach. In this article, it is discussed how the physicians with different clinical specialty interact in the diagnosis and treatment of DF, boarding the diabetic neuropathy, the diagnosis procedures, the pharmacological treatment, and the occurrence of infections caused by fungi and by bacteria. It is also discussed the correct, rational use of antibiotics and the surgical and clinical procedures for each individual case, according to the type of injury, the current diagnosis and treatment of vascular peripherical insufficiency and its differential diagnosis as well as its medical and surgical treatment. As an extreme case of diabetic neuropathy is considered the Charcot’s foot. Another discussed topic in this article is the orthopodologic treatment and prevention and education of patients presenting high risk for DF.
