Biliopancreatic malignant tumors: diagnosis and therapeutics by means of endoscopic retrograde cholangiopancreatography

ABSTRACT
Background: biliopancreatic malignant tumors are not very frequent and have bad prognosis. Endoscopic retrograde cholangiopancreatography constitutes
the most important diagnostic and therapeutic option for advanced stages.
Objective: to describe the diagnosis and therapeutics of biliopancreatic malignant tumors by means of endoscopic retrograde cholangiopancreatography
at Manuel Ascunce Domenech University Hospital, from January 2005 to July 2010.
Methods: a cross-sectional descriptive study was conducted. The universe was composed of 600 patients who underwent endoscopic retrograde
cholangiopancreatography. The sample was made up of 75 cases with diagnosis of malignant tumor of bile ducts or pancreas, was selected through a
non-probabilistic sample. The information of the endoscopic retrograde cholangiopancreatography registrations was collected in a form and was processed
by means of SPSS statistical system.
Results: the age group of 70-79 years (34.6 %) and the male sex (62.7 %) predominated. The 90.6 % of patients presented jaundice. Computed axial
tomography provided positive data in 72.4 % of cases, while ultrasound did it in 67.6 %. The 45.3 % of the diagnoses obtained through endoscopic
retrograde cholangiopancreatography were pancreatic tumors. Sphincterectomy and the placing of endoprosthesis were performed in the 17.3 % and 16 %
of the cases, respectively. Pancreatitis prevailed as a complication (2.6%).
Conclusions: biliopancreatic malignant tumors are more frequent in patients older than 70 years and in men. Neoplasias of the head of pancreas are
the most frequent within this group and the endoscopic retrograde cholangiopancreatography constitutes the main study for the diagnosis and palliative
treatment of these affections, with few complications associated.
