Utility of diagnostic tests in tuberculosis of the otolaryngology and the head and neck region

ABSTRACT
Background
Clinical presentation of tuberculosis in head and neck now differs
from the classically described forms. Given the low bacillary burden
in extrapulmonary forms its diagnosis can be elusive.
Objectives
To describe the clinical presentation of a series of cases of patients
with infection by Mycobacteria in head and neck and to assess the
relative utility of the diagnostic tests employed.
Material and method
We reviewed the clinical records of patients with extrapulmonary
tuberculosis.
Results
We evaluated 33 cases of patients with tuberculosis in head and
neck. We observed cervical adenopathy in 73%, three cases with otic
location, three cases at the larynx, one with a thyroid location, one
with nasal location, and another at the palate.
Conclusion
We should raise the clinical suspicion in cases of chronic or recurring
processes without adequate response to therapy or presenting with
fistula, granulation tissue, or ulceration, especially in patients with
some form of immunosuppression or history of contact with tuberculosis.
Cases with acute presentation are exceptional but should be
considered. We propose a diagnostic protocol in cases of suspicion of
tuberculosis centered on the identification of granulomatous inflammation
(biopsy) or the presence of Mycobacteria (staining or culture).
