RS3PE Syndrome in an Elderly as a Paraneoplastic Syndrome of Colon Cancer

ABSTRACT
Ninety one years old male with diabetes mellitus, chronic obstructive pulmonary disease, ischemic heart disease, and pancreatic cysts. In March 2012 he presents inflammation and erythema of his left hand. At examination, sinovitis of carpal and distal interphalangeal joints, and pitting edema on left hand dorsum and phalanges. ESR 60 mm/hr (1-7 mm/hr), rheumathoid factor 28.9 U/L (0-20 U/L), ani-cyclic citrullinated peptide ‹16 U (‹20 U), hands radiography with edema of soft tissue, without erosions. Rheumathology diagnoses RS3PE. He presents lower gastrointestinal bleeding, so a colonoscopy is done with biopsy, diagnosing invasor anal sac adenocarcinoma, treated with hemicolectomy and colostomy, with complete resection of the cancer. He is managed with deflazacort, and rehabilitation, with good response.REFERENCES
