Análisis y evaluación costos de hospitalización y atención obstétrica en adolescentes embarazadas

ABSTRACT

The cost of obstetric care for pregnant adolescents is uncertain derived from the lack of information on the additional cost that these high risk pregnancies bring about. Objetive: To determine the cost of hospitalization and obstetric care in pregnant teenagers. Material and methods: Retrospective costing study carried out at the Perinatal Maternal Hospital "Mónica Pretelini Sáenz" in Toluca, State of Mexico. From January to December 2010, 64 pregnant patients were analyzed. Two groups were formed, the adolescents group (15 to 19 years old) and the adults one (20-35 years old). The obstetric resolution via (ORV) was calculated considering the caesarean unitary cost in $7, 682. 50 pesos, and $5,267.00 pesos the delivery cost, as well as the hospitalization cost (HC) and the total cost (TC). Association measures and student's t were used in the analisis, seeking to establish a statistical association with a p-value of ‹ 0.05, coupled with horizontal and vertical cost analysis. Results: 10 (31%) adults and 17 (53%) adolescents, out of the 64 studied patients, had caesarean section. Hospitalization day average was 1.8 (± 1.3) in the adults and 2.9 (± 2.1) in the adolescents. Significant differences were found between the cost of the HC and the ORV (p = 0.076 and p = 0. 001 respectively). The risk of having an ORV and an HC cost was 1.6 and 2.6 times; for the TC the risk was 1.2 in adolescents. An increase in the HC in adolescents compared with the adult women (61%) and 18% in the TC was reported. Conclusion: The costs of hospitalization and obstetric care in female adolescents are higher than in adults up to 61% and 18% respectively.
