ABSTRACT

Male 56 years old patient, who initiates clinical pattern in December, 1994, with declining sensitivity in his left hand’s 2nd, 3rd, and 4th fingers, and the addition of paresthesias, myalgias and occasional pain along his left upper limb, becoming progressive with decreased capillary and Raynaud’s phenomenon, and further formation of blisters and discharge of purulent, foul-odor liquid. The diagnosis was made of gangrenous pyoderma. Left axillary and subclavian thrombosis was found on performing a phlebography. The arteriographies of the left upper limb showed considerable ischemia at the level of the hand, whereas the right upper limb had normal findings, and the right lower limb showed blood flow decrease in the leg and foot. The decision was made to amputate the distal and medial phalanges of the 2nd to 5th fingers, as the necrosis went on advancing despite the treatment. The histopathological diagnosis was compatible with Buerger’s disease, which is an uncommon occlusive inflammatory illness, involving the limbs’ small to medium lumen arteries and veins, affecting patients with a history of tobacco smoking.
